What error do you get in the console ?

there is no error at console.. btw running codoforum v3.4 at IIS w/ PHP v5.6.16 on intranet

Can you check if the images are uploaded properly ?

you should be able to see the forum images in codoforum/sites/default/assets/img/attachments

as the image illustrate^ the images are there (appear on preview)

It is probably your server filtering html tags.

If you are using Apache, then mod_security might be behind this problem.
